RFF exists to strengthen heart health and support individuals and families impacted by stroke and cardiovascular disease. We fund initiatives that improve prevention, increase access to critical care, and provide meaningful, long-term support for those navigating life before, during, and after a cardiac or neurological event. Our work is informed by lived experience and grounded in a commitment to dignity, education, and continuity of care.
Stroke and heart disease remain among the leading causes of death and long-term disability, yet many patients and families face gaps in education, advocacy, and post-event support. Survivors often leave acute care without the tools, resources, or guidance needed to fully understand their condition or navigate recovery.
This pillar invests in programs that advance awareness, improve access to care, and strengthen support systems across the full continuum of heart and stroke health—from prevention and early intervention to rehabilitation and long-term quality of life.
